COVID Vaccination Requirement for Extracurricular Activities (Updated 10/25/21)

Dear Los Angeles Unified Family--
We hope that you and your loved ones are safe and well. You are receiving this letter because your eligible high school student(s) participate in athletics, drill or band and the record of their vaccination has not been uploaded into the Daily Pass. In order for them to continue participating in these activities, we would appreciate your support with uploading your student’s proof of vaccination into the Daily Pass by October 31, 2021. The upload process is as follows:

  • The Daily Pass can be accessed via a phone, laptop or tablet by visiting
  • You can upload the proof of vaccination by logging into your Daily Pass account using your Parent Portal email and password or you can use your student’s Daily Pass account.

We would like all students to continue participating in their athletic, drill or band program (both practices and competition). We understand that some students may have an approved medical exemption or qualify for conditional admissions (enrolled as homeless, migrant, foster, special education or military families). Medical exemptions must be uploaded onto the Daily Pass. Conditional admissions require no action from the family, as these students are already enrolled.

Los Angeles Unified is committed to providing your child with the best possible education in the safest possible environment. We want students to have a healthy season of their activities; vaccines are a crucial part of this effort.
